Job summary

Amazing Care Home Health Services is seeking an Intake Coordinator to support our growing Intake team. You will manage a high volume of referrals, collect and verify client information, and coordinate with internal teams to ensure timely onboarding and service delivery.

This role requires attention to detail, the ability to handle competing priorities, and a track record of meeting KPIs in a fast-paced environment. On-site role with weekly pay and growth opportunities.

Qualifications

• 2+ years in a metrics-driven, high-volume role such as intake, admissions, recruiting, or call center.
• Experience with payer/Medicaid portals and referral portals.
• Experience using EMR/CRM systems and Microsoft Office.

Responsibilities

• Manage a high volume of incoming referrals and create client profiles in the EMR system.
• Meet intake performance KPI targets for volume, turnaround time, accuracy, and follow-through.
• Prioritize/triage referrals based on urgency and service requirements for timely onboarding.
• Collaborate with clinical managers and field staff to align referrals with staffing and capacity.
• Collect, verify, and document client information to ensure complete intake records.
• Verify client eligibility/benefits per payer guidelines; escalate discrepancies.
• Maintain professional communication with referral sources and internal stakeholders.
