Job summary

CareAparent is seeking a full-time Intake Coordinator II to coordinate referrals, admissions, medical orders, and insurance verifications to ensure delivery of quality home health care. This role serves as a liaison between referral sources, clients and CareAparent staff, supporting timely communication and care coordination during standard business hours, Monday through Friday.

The ideal candidate has healthcare intake experience, familiarity with EMR systems, and knowledge of insurance payor

Qualifications

• Experience in healthcare intake, admissions, referral coordination.
• Medicare intake experience preferred.
• Home care experience preferred.
• Familiarity with medical terminology.

Responsibilities

• Coordinate referrals, admissions, and medical orders.
• Verify insurance coverage and authorizations.
• Input client data into EMR and tracking systems.
• Communicate with referral sources and clients about status.
• Assist scheduling clinicians and on-call duties.
