Innovive Health is seeking an Intake Coordinator to manage end-to-end referral processes for home health services. You will be the key contact between referral sources, patients, clinicians, and internal teams, handling high-volume communications, verifying insurance, collecting documentation, and accurately entering data to ensure timely care coordination.

Ideal candidates will have strong organizational skills, attention to detail, and ability to collaborate in a fast-paced environment.

Formación

• Associate’s degree or equivalent in related work experience.
• 1+ year experience in a healthcare administrative role.
• Knowledge of medical terminology.
• Proficiency in Windows environment, with emphasis on Word and Excel.

Responsabilidades

• Answers incoming calls for intake of new referrals, resumes of care, or readmissions.
• Collects demographic and insurance information to ensure regulatory compliance.
• Data enters patient information into referral tools and EMR systems.
• Makes outbound calls to referral sources and facilities to obtain required documents.
• Collaborates with clinicians to ensure timely adherence to patient care needs.
• Monitors referrals and coordinates between intake, clinical, and referral teams.
