Amazing Care Home Health Services is seeking a highly organized Intake Coordinator to support our growing Intake team in Aurora. You will manage referrals, verify client information, and coordinate with internal teams for timely onboarding and service delivery.

This role requires attention to detail, ability to handle competing priorities, and growth opportunities in a fast-paced, metrics-driven environment.

Qualifications

• 2+ years in a metrics-driven, high-volume role with clear accountability.
• Experience with payer portals and referral portals such as Epic/Repisodic/Wellsky.
• Microsoft Office proficiency (Teams, Outlook, Excel).
• Experience with EMR/ATS/CRM systems and documenting records.
• Strong communication and ability to document time-sensitive information.
• Ability to manage fast-paced workflows and meet KPIs.
• Detail-oriented and organized with minimal supervision.

Responsibilities

• Manage a high volume of referrals and create client profiles in the EMR.
• Meet defined intake KPIs for volume, turnaround time, accuracy, and follow-through.
• Prioritize/triage referrals by urgency, staffing, and service needs.
• Collaborate with clinical managers and field staff to align referrals with staffing.
• Collect, verify, and document client information (demographics, contact, physician, insurance).
• Verify client eligibility and benefits per payer guidelines and internal processes.
• Maintain professional communication with referral sources and internal stakeholders.
