Job summary

CareRing Health, LLC is seeking an Intake Coordinator to collect admissions data, verify insurance, and coordinate with referral sources. This role supports timely, accurate patient intake and data management in a patient-centered home health environment.

Responsibilities

• Ensure all intake data for admissions requests are collected, completed, and communicated according to SOPs and regulatory requirements.
• Obtain necessary demographic, financial, clinical data via interview with referral agency, patient, or representative.
• Verify patient insurance through payer websites, calls, and other means.
• Perform clerical work (e.g., document management).
• Prepare and maintain patient charts as per non-clinical functions.
• Track all incoming new patients and the status of their paperwork and benefits information.
• Maintain and update EMR platforms in a timely manner.
• Coordinate facilities, hospitals, and referral sources to ensure timely information and scheduling with providers.
• Maintain geographic scheduling for assigned practice/providers.
• Promote efficiency of admission process by answering phones and reporting issues.
• Track all referrals in the system and compile statistics monthly.
