Job Summary:

The Early Intervention (EI) and CPSE Intake Coordinator is responsible for overseeing the intake process for both home-based and school-based early intervention and preschool (CPSE) evaluations and services. The role includes managing inquiries, gathering intake documentation, scheduling evaluations, and ensuring seamless communication among families, service providers, and case managers. This position requires empathy, strong organizational skills, and knowledge of both home and school-based service delivery models.

Key Responsibilities:

  • Initial Contact and Referral Management:
  • Receive and respond to inquiries from families and referral sources seeking EI and/or CPSE services for children, guiding them through the intake process.
  • Provide clear and compassionate guidance to families regarding the evaluation process and next steps.
  • Coordination of Evaluations and IFSP/IEP Meetings:
  • Schedule and coordinate multidisciplinary evaluations (MDEs) for both home-based and school-based services, ensuring all necessary evaluators and team members are involved.
  • Facilitate the scheduling of Individualized Family Service Plan (IFSP) meetings for home-based services or Individualized Education Program (IEP) meetings for school-based services.
  • Ensure that all family members, service coordinators, and relevant providers are notified of the meetings.
  • Documentation and Record Keeping:
  • Ensure all intake forms, medical records, consents, and assessment reports are received, processed, and securely stored.
  • Manage and track intake timelines to ensure compliance with program deadlines, including timely scheduling of evaluations and meetings.
  • Communication and Collaboration:
  • Serve as a liaison between families, schools, service providers, and case managers, ensuring smooth transitions for children entering EI and/or CPSE programs.
  • Compliance and Confidentiality:
  • Ensure all intake procedures comply with state, federal, and local regulations, including HIPAA and FERPA.
  • Ensure confidentiality and privacy are maintained at all times, especially in sensitive family matters.
  • Ongoing Monitoring and Support:
  • Monitor and support families throughout the intake and evaluation process to ensure they feel supported and informed.

Qualifications:

  • High School Diploma or GED required; Associate’s or Bachelor’s degree in human services, social work, psychology, early childhood education, or a related field preferred.
  • Experience in early childhood, special education, social services, or health care is preferred.
  • Knowledge of early intervention services, particularly in both home and school-based settings.
  • Strong communication, organizational, and interpersonal skills.
  • Ability to work with diverse populations and manage sensitive information with confidentiality.
  • Experience in scheduling and coordinating
