Job Overview – Care Coordinator (Community Health):

Compensation: $25/hour

Location: New York, NY

Schedule: Monday to Friday, 9:00 AM to 5:00 PM

Support vulnerable communities across NYC as a Care Coordinator (Community Health) with our client’s outreach team, delivering personalized care planning and resource navigation through in-home visits and field-based services. This mobile role covers Brooklyn, Queens, Manhattan, and the Bronx, helping clients with chronic illness, HIV/AIDS, behavioral health challenges, and housing instability. Use motivational interviewing, EMR platforms, and coordinated care approaches to enhance engagement and improve outcomes within the Health Home model.

Responsibilities as the Care Coordinator (Community Health):

• Care Planning & Assessment: Conduct psychosocial and clinical assessments using HIT platforms to create and update individualized care plans.
• Home Visits & Outreach: Perform regular home visits and borough-wide outreach to support client engagement and monitor care progress.
• Service Coordination: Connect clients with healthcare, housing, mental health, and substance use services, ensuring timely follow-up and linkage.
• Advocacy & Support: Help clients overcome social and systemic barriers while promoting independence through education and empowerment.
• Documentation & Compliance: Complete HML assessments, respond to hospital alerts, and maintain accurate records in EMR per Health Home standards.

Qualifications for the Care Coordinator (Community Health):

• Education: Associate’s degree in Human Services, Social Work, or a related field is required.
• Experience: 2+ years in administrative or direct service roles, preferably with HIV/AIDS, behavioral health, or high-needs populations.
• Technical Skills: Proficient in Microsoft Office and Google Suite, with experience in electronic medical records (EMRs) strongly preferred.
• Industry Knowledge: Understanding of health home care coordination models, harm reduction, behavioral health systems, and social determinants of health.
• Skills & Attributes: Strong communication, organizational, and interpersonal skills with the ability to manage multiple cases in a fast-paced, community-based setting.
• Language Requirement: Bilingual in Spanish and English preferred.
• Additional: Must be willing and able to travel throughout NYC for home visits and client support.
