Job summary

Best Home Health Providers Inc. is seeking an Intake Coordinator to be the first contact for patients, families, and referral partners. You will coordinate care, verify coverage, and set the foundation for a smooth home health experience.

You’ll inform the care team about patient needs, gather essential documentation, and ensure timely transitions from hospital to home. This non-exempt role offers potential for growth and a supportive, family-oriented workplace.

Qualifications

• At least 18 years of age.
• High school diploma required; prior experience in medical administrative support preferred.
• Strong written and verbal communication skills with a professional phone presence.
• Comfort with computers, data entry, and standard office software.
• Friendly, professional demeanor and ability to build relationships with patients, families, and referral partners.
• Strong organization and ability to juggle multiple priorities.
• Reliable personal vehicle and valid driver's license.

Responsibilities

• Evaluate referred patients and gather required documentation including history and physicals and Face-to-Face verifications.
• Verify insurance coverage and obtain prior authorizations for home health services.
• Coordinate with hospital discharge planners, case managers, and clinical teams to support safe, timely transitions home.
• Interview patients and families to understand home needs and build a clear care picture for the team.
• Complete and maintain accurate referral records and intake documentation.
• Serve as a liaison between the organization, hospitals, and insurance providers.
• Track referral data, attend team meetings, and inform leadership of delays in care.
• Perform other duties as required for patient services.
