Caringhand Home Health Solutions, Inc is seeking an Intake Coordinator to guide patients from hospital to home and to verify coverage, coordinating care and setting the foundation for an excellent home health experience.

You will be the first point of contact for patients, families, and referral partners, coordinating care, confirming insurance, and ensuring accurate intake documentation for a smooth transition home.

Competenze

• Must be at least 18 years old.
• High school diploma required; medical admin experience preferred.
• Strong written and verbal communication with a professional phone manner.
• Proficiency in computer use, data entry, and standard office software.
• Friendly, professional demeanor with ability to build relationships with patients and partners.
• Strong organizational skills and ability to manage multiple priorities.
• Valid driver’s license and access to reliable personal vehicle.

Mansioni

• Evaluate referrals and gather required documents including history, physicals, and F2F notes.
• Verify insurance coverage and obtain prior authorizations for home health services.
• Coordinate with discharge planners, case managers, and clinical teams for safe transitions.
• Interview patients and families to assess home care needs for care planning.

• Serve as liaison between the organization, hospitals, and insurers.

Please note that this is a Non-Exempt role at $22 to $25/hr.

Responsibilities

• Evaluate patient referrals and gather required documentation, including history and physical reports and Face-to-Face (F2F) encounter documentation.
• Verify insurance coverage and obtain prior authorizations for home health services.
• Coordinate with hospital discharge planners, case managers, and clinical teams to facilitate safe and timely patient transitions to home.
• Interview patients and families to assess home care needs and provide the care team with relevant information for care planning.
• Complete and maintain accurate referral records and intake documentation.
• Serve as a liaison between the organization, hospitals, and insurance providers to support efficient referral coordination.
• Track referral data, participate in team meetings, and keep leadership informed of delays or barriers to care.
• Perform other duties as assigned/required to support patient services.

• Must be at least 18 years of age.
• High school diploma required; prior experience in medical administrative support preferred.
• Strong written and verbal communication skills, including a professional and courteous phone manner.
• Proficiency in computer use, data entry, and standard office software.
• Friendly, professional demeanor with the ability to build and maintain positive relationships with patients, families, and referral partners.
• Strong organizational skills and the ability to manage multiple priorities effectively.
• Valid driver’s license and access to a reliable personal vehicle.
