Job summary

One Health Home Health is currently seeking an experienced Intake Coordinator to join our team delivering high-quality home-based patient care. You will evaluate referrals, confirm insurance coverage, and coordinate services with clinicians to ensure a seamless continuum of care.

The role requires communicating with physicians, nurses, social workers, and discharge planners, obtaining prior authorizations, and handling fast-paced situations with compassion.

Qualifications

• Minimum of one year of experience in intake coordinating.
• Ability to multi-task in a fast-paced environment.
• Excellent written and verbal communication skills.
• Knowledge of home health management and insurance limitations.
• Fluent English, both verbal and written.

Responsibilities

Job Responsibilities

Evaluates patients referred, identifies insurance coverage, and coordinates referrals for services.

Works with physicians, nurses, social workers and discharge planners to establish a coordinated plan of care.

Obtains and documents prior authorization for services from insurance providers.

Ability to demonstrate compassion and courtesy when dealing with difficult situations.

Capable of working under pressure and time constraints including overtime to complete daily assessments.

Ability to use sound judgment and tactful approach when dealing with co-workers, staff and the public.

Must possess the ability to use and apply clinical knowledge to the referral process.

Required Skills

Minimum of one year of experience in intake coordinating.

Self-starter who enjoys working in a fast-paced environment with the ability to multi-task.

Excellent problem-solving, observation, written and verbal communication skills.

Knowledge of overall home health management including admission criteria, nursing process and insurance limitations.

Basic knowledge of insurances including Medicare/Medi-Cal and HMO limitations.

Effective interpersonal communication skills.

Fluent knowledge of English, both verbal and written.

Job Type: Full‑time

Pay: $22.00 - $26.00 per hour
