As a Home Health Intake Coordinator with Home Health Care, Inc. , you'll be one of the first people responsible for turning a new referral into an organized plan of care.
You'll help move patients from “referred” to “ready for care.”
Great intake coordinators know how to manage a lot of moving pieces without losing sight of the patient behind them.
You'll communicate with hospitals, physicians, referral partners, clinicians, patients, and families to make sure we have what we need to begin care.
Your day may include:
Every detail you handle correctly helps a clinician get through the patient's door faster.
Intake isn't a position where everything arrives neatly one task at a time.
