Job summary

Elite Home Health Care in New York is seeking an Intake Manager to own the referral-to-admission process and lead the intake team. You will coordinate between Intake, Marketing, Enrollment, Eligibility & Authorization, and Coordination to ensure smooth transitions from first contact to start of care.

You will oversee eligibility checks, documentation standards, and staff training while driving improvements in referral turnaround, conversion rates, and overall intake accuracy.

Qualifications

• Minimum of 2 years of experience in home care intake or enrollment.
• At least 1 year in a supervisory or management role.
• Strong knowledge of NYS home care, including MLTC, CHHA, Medicaid, and Medicare intake and eligibility processes.
• Experience with HHAeXchange required; ePaces preferred.

Responsibilities

• Own the intake process end to end, from initial contact through admission and handoff to Coordination.
• Make sure every referral is followed up on, tracked, and closed out with a documented outcome.
• Guide clients through the process of obtaining home care services and help the team do the same.
• Oversee access to CHHA, MLTC, and PCA services; educate clients on eligibility criteria and options.
• Oversee initial assessments to determine client needs and appropriate level of care.
• Review intake files for accuracy, completeness, and compliance with HIPAA, Medicare, Medicaid, NYS DOH LHCSA requirements.
• Identify and correct documentation issues; retrain staff where errors repeat.
• Maintain and update intake policies, procedures, call scripts, and checklists.
• Work with Enrollment, Eligibility & Authorization unit; complete intake action on fast-track referrals and others.
• Partner with Marketing and Business Development on referral sources and updates.
• Hand off complete admissions to Coordination and Staffing for care assignment.
• Serve as escalation point for complex referrals and workflow bottlenecks.
• Track and report intake performance: referral volume, conversion rate, time, and errors.
