The role reports to the Clinical Manager or DON and focuses on intake processing, eligibility verification, and documentation compliance within Medicare and state guidelines.

Qualifikationen

• Associate’s or Bachelor’s degree in healthcare administration, business, or a related field.
• Two years of healthcare intake, admissions, scheduling, or home health operations.
• Experience with EMR systems such as Axxess, WellSky, or Kinnser preferred.
• Strong organizational and communication skills.
• Ability to pass background screening and all compliance requirements.

Aufgaben

• Process new patient referrals from hospitals, physicians, facilities, and partners.
• Verify required patient demographic, insurance, and clinical information.
• Enter referral information into the EMR system accurately and timely.
• Coordinate initial patient visits with clinical staff and ensure documentation is obtained prior to admission.
• Verify insurance coverage and assist with authorizations; communicate status to teams.
