Qualifications

• RN license active in Connecticut; current BLS certification.

Responsibilities

• Oversees intake and referral management, coordinating transition from hospital to home health services.
• Participates in SNF discharge planning meetings and patient conferences.
• Recruits, orients, and evaluates home care intake staff and per diem nurses.
• Collaborates with hospital care coordination and physician practices to manage at-risk patients.
• Develops and evaluates ongoing education programs to improve staff skills and critical thinking.
• Ensures orientation and competency programs meet standards and QA expectations.
• Performs supervisory visits to patient homes during orientation and training.
• Rotates weekend supervisory coverage and participates in QAPI activities.
• Contributes to program planning, budgeting, and evaluation of clinical services.
• Performs other duties as assigned.
