Qualifications

• Experience as Intake Coordinator in LCHSA/MLTC or related healthcare setting.
• Strong understanding of MLTC enrollment and Medicaid eligibility requirements (Medicaid applications not done in-house).
• Knowledge of home care authorization processes and payer requirements.
• Excellent follow-up and ability to manage multiple referrals.

Responsibilities

• Manage referrals through admission and start of care.
• Verify eligibility, authorizations, and payer requirements for home care services.
• Coordinate communication between referral sources, patients, caregivers, and staff.
• Obtain and review all required admission documentation.
• Ensure compliance with NYS Department of Health regulations and policies.
