Job summary

IPC Health Ltd. is seeking an Intake Worker to be the first point of contact for consumers seeking access to Community Health services.

You will complete holistic initial needs screening and determine eligibility to prioritise entry into IPC Health services and external services. You will work with a diverse client base, including those with mental health, family violence, and substance use concerns, and collaborate with a multidisciplinary team to coordinate client care and smooth service

Qualifications

• Tertiary qualifications in health, community services or related field.
• Experience with intake and screening activities in community health.
• Ability to coordinate client care with a multidisciplinary team.

Responsibilities

• Execute holistic initial needs screenings to identify client needs and eligibility.
• Prioritise entry into IPC Health services or external services as appropriate.
• Liaise with diverse clients and coordinate care across services.
