Job summary

Cognitive Consultants LLC is seeking an Intake Specialist to serve as the first point of contact for clients seeking services. You will screen referrals, collect and verify information, assess eligibility, and connect clients to appropriate programs while maintaining confidentiality.

Responsibilities include conducting biopsychosocial assessments, entering data into the case management system, and coordinating with internal and external partners to ensure timely access to services.

Qualifications

• High school diploma or equivalent required; associate's degree in social services, human services, or related field preferred.
• 1+ year of experience in client intake, customer service, social services, or a related setting preferred.
• Strong verbal and written communication skills.
• Basic computer skills; experience with case management or ERM systems preferred.
• Ability to handle sensitive information and maintain professional boundaries.

Responsibilities

• Screen referrals and collect/verify client information.
• Assess initial eligibility and connect clients to appropriate programs and resources.
• Conduct biopsychosocial assessments and develop individualized treatment plans.
• Triage and refer clients to internal programs or external partners; schedule follow-ups.
• Maintain client confidentiality and comply with HIPAA/GDPR as applicable.
• Prepare intake documentation and releases; communicate with probation officers regarding enrollment.
