Job summary

IPC Health Ltd. is seeking an Intake Worker to be the first point of contact for consumers seeking access to Community Health services.

You will complete holistic initial needs screening and determine eligibility to prioritise entry into IPC Health services and external services. You will work with a diverse client base, including those with mental health, family violence, and substance use concerns, and collaborate with a multidisciplinary team to coordinate client care and smooth service

Job description

  • Full time - Fixed term opportunity available to 31 December 2027 (Full time and Part time applications will be considered).
  • Classified as Level 4 Pay point 1-5 (SL4P 1-5) under the Community Health Centre (Stand Alone Services) Social and Community Service Employees Multi-Enterprise Agreement 2022,

  • Assist clients to navigate the service system and facilitate connections with relevant services and support in the community to support their health and wellbeing.
The Opportunity

As an Intake Worker, you will be the first point of contact for consumers seeking access to Community Health services. Within this role, you will complete holistic initial needs screening to identify the client's needs, assessing eligibility using clinical risk maps / prioritisation tools; and prioritise entry into appropriate IPC Health services and/or external services. You will be working with a diverse group of consumers including those with culturally and linguistically diverse backgrounds, who may experience an array of health and wellbeing conditions, including but not limited to, physical health, mental health, family violence, suicidal risks and alcohol and substance abuse. In addition, you will have the opportunity to liaise with other service providers within a multidisciplinary team in assisting with coordinating client care.

You will make a difference by
  • Undertake Initial Needs Identification in a holistic manner; considering the consumers' social, psychological, medical and physical aspects of health; opportunities for health promotion and, if appropriate, exploring issues other than presenting issues.
  • Provide current, appropriate and detailed information about services, organisational waiting list policy and community resources, and how to access them.
  • Possess a broad knowledge of the service sector in the Western Metropolitan Region and the ability to liaise across the full spectrum of health and well-being services.
  • Ensuring smooth transitions for the clients in their journey including booking appointments where availability allows.
To succeed you will need (Key selection criteria)
  • Tertiary qualifications in health, community services, behavioural/social sciences disciplines with demonstrated experience in a similar role within community health or a community-based setting.
  • Experience in undertaking intake and screening activities, such as comprehensive biopsycho-social assessment to effectively determine client needs and assist or refer as needed.
  • Capacity to work independently and in consultation with a multidisciplinary team to coordinate client care.
  • Broad knowledge of the service sector in the Western Metropolitan Region and the ability to liaise across the full spectrum of health and welfare services.
  • Excellent communication and written skills to provide reliable information to guide individuals, families and carers to explore options, make informed decisions, and to facilitate access to services and support both internally and externally to IPC Health.
