Job summary

Advocate Health Care in Mount Holly, NC, is seeking a Physical Therapist to provide direct inpatient rehabilitation services. Qualifications

• Bachelor's degree in Physical Therapy required.
• Applicable state license for Physical Therapy required.
• BLS certification required per policy guidelines.

Responsibilities

• Provides direct therapy services to patients in inpatient rehab.
• Evaluates patients and establishes appropriate patient care plans with modalities and exercise programs.
• Documents evaluations, treatment notes, progress, discharge notes and charges.
• Communicates progress to physicians, referral sources and staff; coordinates schedule and discharge planning.
• Supervises PTA's and Rehab Tech/Aide as required for licensure.
