Responsibilities

• Provides direct therapy services to patients admitted to a level of care requiring service 7 days a week, including holidays.
• Evaluates patients and establishes appropriate care plans including modalities, exercise programs, groups and education.
• Gathers and interprets data from medical records, physician reports, patient and family; documents accordingly.
• Documents evaluations, tests, progress notes, treatment notes, education, outcomes, discharge notes and charges as needed.
• Communicates patient evaluations and progress to physicians, referral sources and other clinical staff; coordinates schedule, treatment programs and discharge planning.
• Provides supervision of PTAs and Rehab Tech/Aide as required for licensure
