Job summary

Advocate Health is hiring a Full-Time Physical Therapist in Clemmons, NC to join the Outpatient Rehabilitation team. You’ll provide patient assessments, develop treatment plans, and support functional recovery for diverse populations in a collaborative setting.

Licensed in North Carolina with current licensure and BS degree in Physical Therapy are required, CPR certification every two years, and a commitment to teamwork.

Qualifications

• Licensed to practice Physical Therapy in the State of North Carolina with current licensure.
• Bachelor of Science degree from an accredited physical therapy school required.
• CPR certification required every two years.

Responsibilities

• Provides patient care assessment and treatment, ensuring evaluations and treatments are comprehensive and goal-directed.
• Maintains accurate patient documentation including evaluations, progress notes, and discharge summaries.
• Meets department financial and productivity standards and manages patient scheduling.
• Participates in process improvements to optimize quality and efficiency of services.
• Supervises students to develop clinical skills and ensure delivery of quality care.
• Communicates professionally with patients, physicians and staff.
