Advocate Health seeks a PRN Physical Therapist for inpatient rehabilitation locations in North Carolina (Carolinas Rehabilitation Northeast or Mount Holly). The role involves evaluating patients, creating care plans, and educating patients and families.

Responsibilities include data gathering from medical records, documenting progress, re-evaluating goals, coordinating discharge planning, and supervising PT aides and students.

Qualifications

• Bachelor's degree in PT from an accredited program.
• Applicable state PT license required.
• CPR and BLS certification per policy guidelines.

Responsabilités

• Evaluate patients and create individualized care plans.
• Gather data from records and physicians to justify treatment.
• Document evaluations, treatment notes, progress, and charges.
• Re-evaluate and adjust goals as patient status changes.
• Communicate progress to physicians and coordinate schedules and discharge plans.
• Supervise PTA, technicians, students, and volunteers; assist with training.
