Job summary

The Social Worker position at Alliance for Care at Home provides comprehensive psychosocial assessments to members and supports their ability to direct their own health care experience through assessment, intervention, and linkage to resources.

Responsibilities include coordinating with Kaiser Permanente and community resources, supporting culturally sensitive care, and documenting plans of care while collaborating with multi-disciplinary teams to improve patient outcomes.

Qualifications

• Minimum one year of social work experience in a hospital, medical outpatient clinic, home health/hospice, or long-term care setting or completion of Kaiser Permanente NW Social Worker Internship program.
• Minimum three years of additional related experience.
• Master's degree in social work from an accredited university.
• Credentials from multiple states required; primary credentials at hire and secondary credentials post-hire.

Responsibilities

• Uses age-appropriate assessment and intervention skills to address patient/family needs.
• Assists members in developing cost-effective, appropriate health care plans leveraging personal resources and Kaiser Permanente programs.
• Promotes multidisciplinary, patient-centered care considering cultural, economic, and personal factors.
• Maintains knowledge of Kaiser Permanente services/benefits, resources, and applicable laws affecting planning.
• Completes required documentation of services and social work plan of care per policy.
• Participates in program development, orientation, and education to enhance members’ ability to direct their health care.
