Job summary

NALHE is seeking a social worker to provide comprehensive psychosocial assessments, care planning, and case coordination for patients and families. The role serves as a liaison between Kaiser Permanente and the community to coordinate care and strengthen community relationships.

Responsibilities include developing and implementing social work plans, conducting assessments, coordinating services, and participating in department activities to support quality improvement and staff education.

Qualifications

• Prepares and evaluates assessments and development of social work service plans.
• Provides psychosocial assessment, diagnostic impressions, treatment of individuals/couples/families/groups, and evaluation of practice effectiveness.
• Applies knowledge base, theories, and essential social values to effect change in behavior and social conditions.
• Helps enhance or restore personal and social functioning.
• Performs case management, coordination, and monitoring of service plans.
• Provides information, resource identification, referrals, mediation, advocacy, and education of individuals and groups.
• Plans, implements, and evaluates social work programs and policies; participates in QA initiatives.

Responsibilities

• Prepares and evaluates social work assessments and care plans.
• Manages case coordination, referrals, and placement for services.
• Assists with program administration and quality improvement activities.
• Conducts records audits and ensures documentation meets standards.
• Participates in staff meetings, in-services, and committees.
• Provides staff consultation and liaison with health care partners.
