Qualifikationen

• Minimum one year of social work experience in a hospital, medical outpatient clinic, home health/hospice, or long-term care setting or successful completion of Kaiser Permanente NW Social Worker Internship.
• Minimum three years of additional experience in a related environment.
• Credentials from multiple states required at hire; specific state licenses and certifications must be obtained within designated timelines.
• Clinical Social Work Associate Certificate (Oregon) within 6 months of hire or LCSW or MSW license (Oregon) within 6 months of hire.
• Washington state social work licenses/certifications within 6 months of hire; National Provider Identifier required at hire.
• Basic Life Support (BLS) within 3 months of hire.

• Uses age-appropriate assessment and intervention skills.
• Assists members in developing cost-effective, appropriate health care plans using Kaiser resources and community programs.
• Promotes multidisciplinary, patient-centered care considering personal, psychological, economic, and cultural needs.
• Maintains knowledge of Kaiser Permanente services, public/private resources, and applicable laws.
• Completes required documentation of services and the social work plan of care per policy.
• Participates in program development, orientation, and educational activities to empower members.
