Responsibilities

• Progress Monitoring & Evaluation: Track progress and adjust care plans with the care team.
• Advocacy: Promote patient access to appropriate treatment and communicate with the care team.
• Active Listening: Provide a safe, non-judgmental space for patient expression.
• Respect for Individuality: Honor cultural, spiritual, and personal backgrounds in care.
• Building Trust: Develop trust-based relationships with patients and families.
• Risk Segmentation: Identify patients needing extra support and prioritize interventions.
• Tasks or Interventions: Carry out delegated tasks as part of the care plan.
• Delegated Work: Coordinate referrals and follow-up assessments with the team.
