Qualifications

• Master's degree in social work from an accredited university.
• Minimum 1 year hospital/medical social work experience, or completion of Kaiser internship.
• Licensing credentials required from primary state at hire; additional state credentials may be needed post hire.
• BLS certification within 3 months of hire.

Responsibilities

• Uses age appropriate assessment and intervention skills.
• Assists members in developing cost effective, appropriate health care plans through optimizing resources.
• Promotes multi-disciplinary care that is patient-centered and considers psychological, economic, and cultural needs.
• Maintains knowledge of Kaiser Permanente services/benefits and applicable laws.
• Completes documentation of services and Social Work plan of care per department policy.
• Participates in program development, orientation, and educational activities.
