Qualifications

• Master of Social Work degree from a CSWE-accredited program.
• Current licensure in the practicing state (or as required by OSH Center).
• Fluency in Spanish, Polish, Russian, or other languages used in the communities served.

Responsabilités

• Assess and coordinate care for high-need patients and caregivers.
• Collaborate with interdisciplinary team to develop and implement care plans.
• Refer to community resources and navigate insurance benefits with the care team.
• Provide psychosocial support and crisis intervention as needed.
• Document interventions and outcomes in patient records.
• Engage with patients and families to educate on care options and goals.
