Kaiser Permanente NW is seeking a Social Worker to provide comprehensive psychosocial assessments and support members in directing their health care. You will collaborate with medical teams, link to resources, and promote culturally respectful care across diverse populations.

The role requires licensure across states, a Master’s in social work, and the ability to work in a multidisciplinary setting. BLS and NPI are required within specified timeframes.

Competenze

• Experience: 1 year in hospital/clinic/home health/hospice/long-term care or completion of KP NW Social Worker Internship.
• 3+ years of additional related experience.

• Uses age-appropriate assessment and intervention skills.
• Assists members in developing cost effective, appropriate health care plans.
• Promotes multidisciplinary, patient-centered care considering personal, psychological, economic and cultural needs.
• Maintains knowledge of Kaiser Permanente services/benefits and community resources.
• Completes documentation of services and social work plan of care.
• Participates in program development, orientation, and education to empower members.
