Job summary

Qualifications

• Master's Level Degree in Social Work (MSW) required.
• 1 year of acute care and/or healthcare experience preferred.
• BLS certification within 90 days of hire; CM certifications within one year.

Responsibilities

• Provide medical social work services to hospital patients.
• Assess social, environmental, and financial needs and plan transitions.
• Collaborate with healthcare team and community resources for disposition and discharge.
• Coordinate with RN Case Manager to ensure smooth patient flow through stays.
