Job summary

Sentara Princess Anne Hospital is seeking an Inpatient Case Manager/Social Worker (MSW). The role is full-time from 8:00 AM to 4:30 PM EST, Monday–Friday, with rotating weekends. onsite at 2025 Glenn Mitchell Drive, Virginia Beach, VA 23456.

A $15,000 sign-on bonus is available for qualified candidates and a comprehensive benefits package is provided. The ideal candidate has an MSW, BLS within 90 days, and case management certification within a year.

Qualifications

• Master's Level Degree in Social Work (MSW) is REQUIRED.
• Basic Life Support (BLS) Certification within 90 days of hire is REQUIRED.
• Case Management certifications (ACM, CCM, CCCTM, CMAC, or CMGT-BC) REQUIRED within 1 year of eligibility.

Responsibilities

• Provide medical social work services to hospital patients.
• Assess social, cultural, environmental, and financial situations and disposition needs.
• Collaborate with healthcare team and community resources for patient needs.
• Develop effective transition plans with the healthcare team.
