Job summary

ChenMed is seeking a Primary Care Physician (PCP) to join our expanding clinical operations team. This licensed/Board Certified/Board Eligible professional will provide direct patient care in ambulatory health centers, with potential duties in other care settings as needed.

The PCP will conduct comprehensive assessments, develop care plans, coordinate referrals, and participate in rounds and conferences while documenting thoroughly to ChenMed standards for quality, service and teamwork.

Responsibilities

• Functions independently as a primary care practitioner as part of a patient care team.
• Independently assesses acute and non-acute clinical problems.
• Performs and documents physical assessments and patient histories, analyzes trends in patient conditions, and develops, documents and implements a patient management plan.
• Plans patient care based on in-depth knowledge of the specific patient population and protocol, anticipating and identifying problems.
• Participates in patient care rounds and conferences. Communicates patient management strategies to the care team.
• Collaborates with multidisciplinary team to ensure patient management strategies meet care needs.
• Recognizes urgent situations and initiates life-saving procedures when necessary.
