You will conduct Health Risk Assessments, identify immediate care gaps, arrange services, and support members in navigating benefits and community resources while traveling to visit enrollees in homes or care facilities as needed. This field role emphasizes proactive outreach, documentation, and collaboration with providers to improve member well-being.

Qualifications

• Must reside in Illinois and have 1+ years of health care or case management experience.
• Experience with Medicare/Medicaid, long-term care, or managed care organizations.
• Intermediate to advanced skills in Word, Excel, Outlook.
• Experience with electronic medical records.

Responsibilities

• Visit Medicaid enrollees in homes or facilities; perform Health Risk Assessments.
• Proactively outreach to members and connect to services.
• Coordinate services and monitor care plans; address barriers to care.
• Assist with grievances and appeals; provide information on benefits.
• Travel up to 75% locally.
