Job summary

OneHome is seeking a licensed Pharmacist to join our infusion-focused team in Florida. You will ensure safe and compliant dispensing of medications and manage sterile compounding, order processing, and inventory according to USP 797/800 and ACHC standards.

The role involves collaborating with care teams, reviewing patient information, monitoring drug levels, and educating staff and caregivers. A state license and 1-2 years of home care or infusion pharmacy experience are preferred.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Active pharmacist license in the state of practice.
• 1-2 years of clinical home care, infusion, or hospital pharmacy experience.
• Knowledge of USP 797/800 guidelines.

Responsibilities

• Maintain sterile IV admixtures and aseptic technique per USP 797/800.
• Assess care plans and tailor them to the patient’s medical condition.
• Collect medical information from caregivers, case managers, physicians, and patients to determine home care suitability.
• Monitor drug levels and lab values, making regimen adjustments in collaboration with the ordering physician, nurse, and patient.
• Educate other staff, patients, and caregivers on pharmacotherapy and safety.
• Ensure ongoing compliance with laws and company policies (DEA, BOP, ACHC).
