Job summary

OneHome seeks a licensed pharmacist to ensure safe and compliant pharmacy services in a home infusion setting. You will oversee sterile compounding, IV admixtures, order processing, and inventory while aligning with USP 797/800 and ACHC standards.

You will collaborate with physicians, nurses, and caregivers, monitor lab values and drug interactions, and participate in quality improvement programs to uphold high patient care standards.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active pharmacist licensure in state of practice.
• 1-2 years of clinical pharmacy experience in home care, hospital, or infusion setting.

Responsibilities

• Maintains sterile IV admixtures and aseptic technique according to USP 797/800.
• Designs and monitors patient-specific care plans.
• Gathers medical information to determine home care suitability per policy.
• Performs pharmaceutical calculations and monitors pharmacokinetics.
• Educates staff and patients on pharmacotherapy.
