Humana Inc. (OneHome) is seeking a licensed Pharmacist to oversee safe and effective dissemination of pharmacy services for home infusion patients.

The role requires active licensure (Pharm.D., Rph) and experience in sterile compounding, regulation knowledge, and patient counseling. Responsibilities include reviewing orders, maintaining quality control, and collaborating with physicians, nurses, and caregivers to optimize pharmacotherapy while ensuring compliance with USP 797/800 and ACHC

Competenze

• Graduate of an accredited School of Pharmacy.
• Current, active pharmacist licensure (Pharm.D., Rph) in state of practice.
• 1-2 years of clinical pharmacy experience in home care, hospital, or infusion setting.

Mansioni

• Maintains sterile IV admixtures knowledge and aseptic technique per USP 797/800.
• Gathers medical information to determine suitability for home care services.
• Monitors laboratory values and makes pharmacokinetic-based adjustments.
• Educates staff and patients on pharmacotherapy and ensures compliance with regulations.
• Ensures ongoing compliance with laws and regulations (DEA, BOP, ACHC).
