Option Care Health in Florida is seeking a Clinical Pharmacist to provide safe and appropriate pharmacy services in the home care setting. This entry-level role collaborates with the Nursing Department, educates patients and caregivers, and ensures compliance with policies and regulatory standards.

You will monitor patient progress toward goals, coordinate with the care team, dispense medications, calibrate equipment, and participate in continuing education to maintain competency.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• One of: sterile IV admixture procedures; non-sterile compounding, immunizations and MTM; ASHP residency; Advanced Pharmacy Practice Experience; or equivalent experience.

Responsibilities

• Determines patient suitability for home care; collects information to determine eligibility; assesses suitability per policy.
• Designs, monitors and evaluates patient-specific pharmacy care plans for home care patients.
• Modifies care plans based on data; assesses labs and uses pharmacokinetic principles.
• Provides medication-use education to home care staff, patients and caregivers.
• Ensures continuity of pharmaceutical care across home care and other settings.
• Manages equipment use, maintenance and troubleshooting related to medication administration.
• Responds to drug information requests with literature-based answers.
• Complies with accreditation, legal, regulatory and safety requirements.
