The role emphasizes accuracy per USP 797/800, collaboration with physicians and nurses, on-call responsibilities, and participation in quality improvement programs.

Applicants should hold Pharm.D. or RPh licensure, with 1–2 years of clinical home-care or infusion pharmacy experience.

Qualifications

• Knowledge of sterile IV admixtures and aseptic technique per USP 797/800.
• Clinical pharmacy experience in home care, hospital, or infusion settings (1–2 years).
• Strong communication with patients, caregivers, and healthcare teams.

Responsibilities

• Maintains knowledge of sterile IV admixtures and aseptic technique per USP 797/800.
• Determines and monitors care plans tailored to patient needs.
• Interacts with physicians, nurses, and patients to optimize pharmacotherapy.
• Monitors lab values and makes regimen adjustments as needed.
• Educates staff and patients on pharmacotherapy and safety.
• Ensures compliance with DEA, BOP, ACHC, and accreditation standards.
