Job summary

InfuCare Rx is seeking a licensed Pharmacist in Florida to join the home infusion team in Boynton Beach. The role involves coordinating with physicians, nurses, and staff to ensure safe drug therapy and accurate orders in a home infusion setting.

You will monitor patient therapies, manage sterile compounding when applicable, and maintain comprehensive documentation while supporting ongoing education and coverage as needed.

Qualifications

• Pharmacist degree (PharmD or BS in Pharmacy) and Florida license.
• Experience in acute-care or home infusion environments preferred.
• Strong knowledge of intravenous admixture and home infusion therapy.

Responsibilities

• Adheres to organizational policies and procedures.
• Coordinate referrals/admissions with nursing, customer service, and distribution.
• Review and verify patient orders for accuracy.
• Collaborate with physicians and nurses to plan patient care.
• Assess drug therapy and monitor safety and efficacy.
• Communicate monitoring results to care teams.
• Sterile compounding oversight and documentation as required.
• Maintain thorough patient records and prepare progress reports.
• Participate in on-call rotation if applicable.
• Support marketing and training activities.
