• Graduate of an accredited School of Pharmacy.
• Current, active pharmacist licensure in state of practice.
• Knowledge of sterile IV admixture and USP 797/800 guidelines.
• 1–2 years of clinical pharmacy experience in home care, hospital, or infusion setting.

Aufgaben

• Maintains sterile IV knowledge and aseptic technique per USP 797/800.
• Designs and monitors patient care plans tailored to medical conditions.
• Gathers medical information from caregivers and physicians to determine home care suitability.
• Monitors lab values and makes pharmacotherapy recommendations.
• Educates staff, patients, and caregivers on pharmacotherapy matters.
• Ensures compliance with laws and regulations (DEA, BOP, ACHC).
• Monitors inventory and on-call responsibilities as per policy.
