Humana’s OneHome infusion pharmacy seeks a licensed Pharmacist to ensure safe dispensing of medications and sterile compounding in line with USP 797/800 and ACHC standards. The role includes patient-centered care planning, interprofessional collaboration, and ongoing compliance with regulatory requirements.

The Pharmacist will monitor medication orders, verify accuracy, counsel patients, and participate in a performance improvement program to enhance care quality and safety across home care

Qualifikationen

• Graduate of an accredited School of Pharmacy.
• Current, active pharmacist licensure (Pharm.D., Rph) in state of practice.
• Demonstrates sufficient knowledge of sterile IV admixture and USP 797/800 guidelines.
• 1-2 years of clinical pharmacy experience in home care, hospital, or other infusion setting.

• Maintains sterile IV admixtures, aseptic technique per USP 797/800.
• Designs and monitors patient care plans.
• Collects medical information to determine suitability for home care services.
• Monitors lab values and provides pharmacotherapy adjustments.
• Educates staff, patients, and caregivers on pharmacotherapy.
