Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active pharmacist licensure in state of practice (Pharm.D., Rph).
• Demonstrates knowledge of sterile IV admixture and USP 797/800 guidelines.
• 1-2 years of clinical pharmacy experience in home care, hospital, or infusion setting.

Responsibilities

• Maintains sterile IV admixtures, aseptic technique, and environmental monitoring per USP 797/800.
• Designs and monitors care plans tailored to patient condition.
• Gathers medical information to determine home care suitability.
• Performs advanced pharmaceutical calculations and monitors lab values.
• Educates staff and patients on pharmacotherapy and safety.
• Ensures compliance with laws and policy, assists in quality improvement.
