Humana is seeking a licensed Pharmacist to oversee safe dispensing of medications for home infusion patients. The role includes verifying sterile IV admixtures, ensuring regulatory compliance, and collaborating with caregivers and clinicians to optimize pharmacotherapy.

The Pharmacist will monitor orders, prepare and dispense medications per USP 797/800, and participate in quality improvement and accreditation activities to maintain high standards of care.

Formación

• Graduate of an accredited School of Pharmacy.
• Current, active pharmacist licensure in state of practice.
• Demonstrates knowledge of sterile IV admixtures and USP 797/800 guidelines.
• 1–2 years of clinical pharmacy experience in home care, hospital, or infusion setting.

Responsabilidades

• Maintains sterile IV admixture knowledge and aseptic technique in accordance with USP 797/800.
• Gathers patient information to determine suitability for home care services.
• Performs pharmaceutical calculations and monitors pharmacotherapy and lab values.
• Educates staff, patients, and caregivers on pharmacotherapy and safety.
• Ensures compliance with DEA, BOP, ACHC and accreditation standards.
• Participates in continuity of care and quality improvement programs.
