InfuCare Rx, a leading home infusion provider in Florida, seeks a licensed Pharmacist to support IV therapy and home infusion services. You will engage with physicians, nurses, staff, insurers, and patients to ensure safe drug therapy and effective home care delivery.

Responsibilities include verifying orders, coordinating admissions, monitoring therapy, and maintaining compliance with pharmacy laws. A strong team player with clinical and distributive pharmacy skills is required.

Qualifications

• Pharmacist with PharmD and current state license.
• Experience in acute-care or home infusion settings preferred.
• Strong knowledge of intravenous admixture and drug therapy management.

Responsibilities

• Adheres to policies and coordinates referrals with nursing and distribution.
• Verifies new patient orders and ensures accuracy on forms.
• Assists physicians and nurses in planning safe drug therapy in home care.
• Monitors drug therapy outcomes using lab and clinical data.
• Communicates therapeutic monitoring results to the care team.
• Prepares sterile admixtures and dispenses pharmaceuticals per laws.
• Maintains thorough documentation in patient records and reports.
