Job summary

Amerita is seeking a Pharmacist to join our pharmacy team for our Pawtucket, RI branch. You will provide pharmaceutical care to home infusion patients and collaborate with physicians, nurses, and distribution staff.

The role requires a Doctor of Pharmacy (or PharmD) with active licensure, experience in infusion therapy or acute care, and ability to work on-call evenings and some weekends. Competitive benefits and on-going professional development are offered.

Qualifications

• Must have Bachelor of Science in Pharmacy; PharmD preferred.
• Licensed Pharmacist in state(s) of practice.
• Minimum 2 years in acute-care or 1 year in home infusion therapy.
• Experience with compounding infused drugs.
• Strong evaluation and monitoring of drug therapies for home care patients.
• Excellent written and verbal communication skills.

Responsibilities

• Evaluates and monitors drug therapy for home care patients in collaboration with physicians, nurses and other healthcare professionals.
• Adheres to organization policies, procedures and state/federal laws governing pharmacy practice.
• Coordinates new referrals and admissions with nurses, customer service and distribution.
• Reviews and verifies new patient orders and documents appropriately.
• Assists physicians and other professionals with home care planning and monitoring.
• Documents and maintains patient records of clinical activities and communications.
• Ensures medications, supplies and equipment are prepared prior to dispensing.
• Maintains compounding areas in compliance with USP 797 and USP 800 standards.
• Oversees pharmacy technician functions as required by Board rules.
