Job summary

Amerita, Inc. is seeking a Pharmacist to join our Home Infusion Pharmacy team. The pharmacist will report to the Pharmacy Manager and work at our City, State branch, delivering pharmaceutical care to patients in the service area.

You will evaluate and monitor drug therapy, ensure compliance with regulations, and coordinate referrals with nurses and care teams. Strong communication and organizational skills are essential.

Qualifications

• Excellent verbal and written communication skills.
• Strong organization and time-management abilities.
• Ability to work on-call evenings/weekends as scheduled.

Responsibilities

• Evaluates and monitors drug therapy for home care patients with physicians and nurses.
• Adheres to policies, procedures, and state/federal laws governing pharmacy practice.
• Coordinates referrals and admissions with care teams and distribution.
• Reviews new patient orders and documents appropriately.
• Assists with planning patient care at home and therapy objectives.
• Assesses drug and disease information to identify problems.
• Monitors therapy outcomes using lab and clinical data.

Education

Bachelor of Science in Pharmacy

Doctor of Pharmacy (preferred)
