Amerita is seeking a skilled Pharmacist to join our home infusion pharmacy team. The role requires evaluating drug therapy, ensuring compliance with USP standards, and coordinating patient care with nurses and physicians in a home setting.

Applicants should have a PharmD or BS in Pharmacy, be licensed, and possess experience in acute care or home infusion, with strong communication and organizational skills and flexibility for on-call shifts.

Qualifications

• Pharmacist with BS Pharmacy required; PharmD preferred.
• Licensed Pharmacist in state(s) of practice.
• Minimum of two years in acute-care setting or one year in home infusion.
• Experience with compounding infused drugs.
• Proven record evaluating and monitoring drug therapies for home care patients.
• Excellent verbal and written communication skills.
• Flexibility to work on-call evenings and weekends.

Responsibilities

• Evaluate and monitor drug therapy for home care patients in collaboration with physicians and nurses.
• Adhere to all policies, procedures and regulations governing pharmacy practice.
• Coordinate referrals and admissions with nurses, customer service and distribution.
• Review and verify all orders and document on the order form.
• Assist physicians and healthcare professionals with patient care planning and monitoring.
