Job summary

Amerita is seeking a Pharmacist to join our Pharmacy team in a home infusion setting. You will evaluate drug therapy, collaborate with physicians and nurses, and ensure compliance with all laws and accreditation standards. The role includes coordinating referrals, reviewing orders, and supporting patient care in the home setting.

The position requires a PharmD or B.S. in Pharmacy with an active pharmacist license.

Qualifications

• Pharmacy degree(s) required or preferred.
• Active pharmacist license in state of practice.
• Experience in home infusion or acute care settings is desirable.
• Flexibility to work on-call evenings and weekends.

Responsibilities

• Evaluate and monitor drug therapy for home care patients with the healthcare team.
• Adhere to all laws, policies, and accreditation standards for pharmacy practice.
• Coordinate referrals and admissions with nurses, service and distribution.
• Review and verify new patient orders and document accordingly.
• Assist physicians and other professionals with home care planning and monitoring.
