Amerita, Inc. is seeking a Pharmacist to join our home infusion pharmacy team. The Pharmacist will report to the Pharmacy Manager and work at the Englewood, CO location, supporting care for home infusion patients.

Ideal candidates have a PharmD and active state license with 2+ years acute-care or 1 year home infusion experience, strong communication, and ability to work an on-call schedule. Competitive benefits offered.

Formación

• Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred.
• Registered Pharmacist License in state(s) of practice.
• Minimum of two (2) years of recent experience in an acute-care setting or one (1) year in home infusion therapy.
• Experience with compounding infused drugs.
• Proven record of evaluating and monitoring drug therapies for home care patients.
• Good organizational and time-management skills.
• Participation and membership in professional societies and organizations is desirable.
• Excellent verbal and written communication skills.
• Flexible schedule with ability to work on-call evenings and weekends as scheduled.
• Travel: 0-25% - minimal travel may be required.

Responsabilidades

• Evaluates and monitors drug therapy relative to home care patient needs in collaboration with physicians, nurses and other healthcare professionals.
• Adheres to organization policies and procedures and all applicable state and federal laws and regulations governing pharmacy practice.
• Participates in quality assurance activities and accreditation standards.
• Coordinates and facilitates new referrals and admissions through collaboration with nurses, customer service and distribution.
• Reviews, receives and verifies all new patient orders and documents on the order form.
• Assists clinicians with planning and implementing home patient care and monitoring therapy.
• Documents initial and ongoing patient, drug and disease information to identify drug-related problems.
• Monitors outcomes of therapy by analyzing laboratory and clinical data.
• Provides consultation to patients, families, physicians and healthcare professionals regarding drugs and their use.
• Maintains thorough documentation in patient records of all clinical activities.
• Processes and checks all medications, supplies and equipment prior to dispensing.
• Maintains skills by reviewing literature and attending trainings.
• Oversees Pharmacy technician functions as required by Pharmacy Board rules.
