Amerita, an affiliate of BrightSpring Health Services, is seeking a Pharmacist to join our home infusion pharmacy team in Rhode Island. The role reports to the Pharmacy Manager and requires clinical care, drug therapy evaluation, and collaboration with physicians, nurses, and distribution teams.

Schedule is Monday–Friday with on-call 2x per month. We offer competitive pay and a full benefits package including health/dental/vision, disability, 401k, tuition reimbursement, and PTO.

Formación

• Pharmacist licensure in state(s) of practice.
• BS in Pharmacy required; PharmD preferred.
• Minimum 2 years in acute-care or 1 year in home infusion.

Responsabilidades

• Evaluate and monitor drug therapy for home care patients in collaboration with physicians and nurses.
• Adhere to all policies, procedures and regulations governing pharmacy practice.
• Coordinate referrals/admissions with nurses, customer service and distribution.
• Review and verify new patient orders and document accordingly.
• Assist physicians and other professionals with planning patient care at home and monitoring therapy.
